Transforming Care Partnership

Barnsley, Wakefield, Kirklees, Huddersfield and Calderdale (CKWB)

The CKWB Transforming Care Partnership’s (TCP) plans focus on developing quality community services with crisis response teams to support people with a learning disability and/or autism.

The TCP will work with people with a learning disability and/or autism and their families and carers to create new types of housing schemes and personalised support packages to improve the quality of support people get and prevent them going into hospital. It will make sure it has the right staff with the right skills to be able to deliver this new type of care and support.
