Heart Failure

Heart Failure - Patient and Carer Survey 2017 

We are currently looking at the way in which local Heart Failure Services are provided.

As part of this work, we have been asking people about their experiences of accessing and using the types of services highlighted below, as the more feedback we can get, the better we can ensure that future services meet the needs of the local people accessing them.

Local Heart Failure Services are provided by the following;

  • GP and or Practice nurse / nurse practitioner
  • Community heart failure nurse
  • Cardiac practitioner – nurse (hospital)         
  • Cardiologist (hospital)
  • Community Matron         
  • Care Navigation service – telehealth
  • Cardiac rehabilitation
  • Local support groups

Thank you to everyone who took the time to provide us with their feedback to the online survey.   The feedback is currently being collated and an update will be provided soon.
